This position operates within a community health clinic setting while allowing for a flexible remote work arrangement. The social worker will collaborate primarily with seniors and their families but will also engage with interdisciplinary teams consisting of healthcare professionals, including nurses and caregivers. This collaborative approach ensures holistic care for each client, aligning our wide-ranging services with specific community health needs.

• Conduct comprehensive assessments of clients' psychosocial needs by evaluating emotional, mental, and social aspects critical to their health.
• Develop tailored care plans in collaboration with clients and families, ensuring each plan addresses individual goals, challenges, and circumstances.
• Provide counseling and emotional support to assist clients through various life transitions, changes in health status, and personal challenges they may face.
• Advocate for clients by facilitating access to necessary community resources and services, ensuring they receive optimal support in all phases of their care.
• Maintain consistent communication with clients' families to educate them on available resources and approaches, ultimately facilitating better engagement in care processes.
• Utilize electronic health record (EHR) systems to document assessments, progress notes, and care plans, ensuring all client interactions are accurately captured for ongoing evaluation.
• Participate in regular interdisciplinary team meetings to share insights, coordinate care strategies, and resolve potential issues affecting client health outcomes.
• Monitor clients through regular visits to assess their living situations, ensuring safety and addressing emerging concerns proactively.
• Stay up-to-date with best practices in social work and policies affecting aging populations to provide informed care that meets ethical and professional standards.

Our primary focus is on the senior population, encompassing elderly clients facing a variety of psychosocial challenges, including mental health issues, social isolation, chronic illnesses, and significant life transitions. The successful candidate will demonstrate an empathetic approach tailored to the unique circumstances of each client, fostering a trusting and secure environment that enhances client engagement.

• A Master’s degree in Social Work (MSW) from an accredited institution.
• Current and valid licensure as a Licensed Master Social Worker (LMSW) in Pennsylvania.
• A minimum of two years of experience in social work, preferably in a geriatric or community health context, demonstrating expertise in serving the needs of older adults.
• Proven track record of utilizing strong assessment abilities, counseling, and advocacy skills to support clients effectively.
• Exceptional interpersonal abilities, showcasing empathy, compassion, and an ability to establish rapport with clients and their families.
• Proficiency in utilizing electronic health record systems and familiar with relevant technology to document and manage client records efficiently.

• Experience working within a community health clinic environment, particularly with senior clients and their families.
• Familiarity with available community resources related to elder care and support services.
• Training in gerontological social work and knowledge of aging-related issues and policies.

The compensation for this position ranges from $60,000 to $100,000 annually, commensurate with experience and skills.
