This position is fully remote, allowing you to provide telehealth services from the comfort of your own home while reaching patients in need across Pomona and its surrounding areas. Our telehealth platform enables seamless communication and interaction with patients, ensuring continuity of care and flexibility for both therapist and client. As part of our multidisciplinary team, you will collaborate with geriatric psychiatrists and other mental health professionals, maintaining a supportive and engaged work atmosphere.

• Conduct thorough mental health assessments that are sensitive to the unique needs and challenges faced by older adults, developing a nuanced understanding of their psychological and emotional states.
• Create and implement customized treatment plans that reflect each patient’s cognitive changes, physical health, and social dynamics, while adhering to best practices in geriatric mental health.
• Utilize evidence-based therapeutic modalities, delivering both individual and family therapy sessions tailored to geriatric psychiatric conditions.
• Engage collaboratively in patient care with geriatric psychiatrists, nurses, and social workers, ensuring a whole-person approach to mental health care.
• Provide crisis intervention and immediate support in mental health emergencies, possessing the skills to maneuver sensitive and critical situations effectively.
• Educate patients, their families, and caregivers about mental health conditions, potential treatment paths, and relevant community resources available to them.
• Maintain accurate, confidential electronic health records in accordance with legal and ethical standards using our state-of-the-art EHR system.
• Participate actively in team meetings and case consultations, contributing to ongoing professional development and staying current with advancements in geriatric psychiatry.

The primary patient population for this role includes older adults experiencing a range of mental health disorders, such as depression, anxiety, and neurocognitive disorders. Recognizing the sensitivity and complexity of their situations, your role will be to provide empathetic support while implementing effective treatment strategies that empower your patients to improve their mental health outcomes.

• A Master’s or Doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.
• Current and valid licensure as an LCSW, LPCC, or LMFT in California.
• A minimum of two years of post-licensure experience is preferred, particularly within a geriatric setting.
• Strong foundation in geriatric mental health disorders and familiarization with evidence-based treatment modalities.
• Demonstrated experience in providing individual and family therapy to older adults, showcasing your ability to connect and communicate effectively.
• Exceptional interpersonal skills, including a genuine patient-centered approach and an empathetic nature essential for building trust with elderly patients and their families.
• Proficient in electronic health record systems and various computer applications necessary for maintaining patient records.
• Ability to work collaboratively as part of a multidisciplinary team, reinforcing a holistic approach to patient care.

• Experience in crisis intervention and managing urgent mental health situations related to geriatric populations.
• Background in community outreach and educational initiatives related to mental health awareness in elder care.
• Familiarity with telehealth protocols and technologies to maximize patient engagement and care delivery.
