Job description

Project Renewal is a leading non-profit organization with the mission of building supportive communities where people achieve dignity and independence, renewing their lives with health, homes, and jobs. Our comprehensive and integrated 24/7 programs are designed to offer the holistic care our clients need. With a staff of 950+ and an annual budget of $140 million, Project Renewal is one of the larger social service nonprofits in New York City.

Title:                   Abstinence Case Manager

Program:           In Homes Now

Salary Range:   $44,000 -$49,500       

Program Description:

In Homes Now is a Scatter Site Housing Program. The Abstinence Program provides housing to clients in recovery who are leaving the shelter system and have histories of homelessness, substance use, and mental health. The Abstinence Program presently houses 85 clients in Brooklyn, The Bronx, and Manhattan. The multidisciplinary team includes case managers, peer counselors, psychiatric care providers, and occupational therapy interns who work with clients to ensure housing stability and improve wellbeing.

Overall Responsibilities:

This is a full-time position at PRI. The case manager will be responsible for managing a caseload of 28-30 clients. The case manager will be responsible for meeting with the clients a minimum of two times per month. This position requires field work.

Essential Duties and Responsibilities:

The essential duties of the Abstinence Case Manager include but are not limited to the following activities: 

  • Conduct monthly home visits to resident apartments.
  • Conduct monthly office visits.
  • Conduct annual housing inspections.
  • Counsel clients about various issues such as money management, sobriety, and employment and other goals
  • Facilitate caseload groups.
  • Maintain accurate and up-to-date clinical documentation and file for caseload
  • Complete initial and annual assessments
  • Complete quarterly service plans
  • Provide referrals to community providers to ensure that clients are linked to providers in their neighborhood.

Qualifications:

  • BA required.
  • A minimum of two years in the human service field
  • Experience working with individuals who are homeless and/or those affected by substance abuse preferred.
  • Excellent verbal and written communication skills
  • Strong time management skills
