Family Nurse Practitioner (APRN) (Full-time; 40 hours/week)

Location: Norwich, CT

Generations Family Health Center, Inc. is a regional community health center covering a thirty seven town rural region in eastern Connecticut with sites located in Willimantic, Danielson, Norwich and Putnam. Our dedicated staff provides a full range of primary care services to all ages including medical, dental and behavioral health care, as well as care coordination and community outreach. In addition to our main facilities, we offer mobile programs and School Based Health Centers.

Family Nurse Practitioner Role: Provide a broad range of clinical primary care services, which will include routine and appropriate ambulatory adult, pediatric, and gynecological care as assigned.

Key responsibilities include:

  • Providing care appropriate to the age of the patients served

  • Performing office diagnostic and therapeutic procedures

  • Performing therapeutic procedures such as injections, immunizations, suturing, wound care, and managing infection

  • Instructing and counseling patients regarding compliance with prescribed therapeutic regimens, normal growth and development, family planning, emotional problems of daily living, and health maintenance

  • Developing and implementing patient management plans, recording progress notes, and assisting in provision of continuity of care

  • Performing comprehensive physical examinations and compiling patient medical data, including health history and physical exam results

Qualifications

Qualifications:

  • M.S. or D.N.P degree from an accredited school of nursing

  • Board certification

  • Current licensure in the State of Connecticut or ability to obtain license in the state

  • Valid Federal and State Controlled Substance License

Compensation: Base salary starting at $116,000+ (compensatory salary based on experience)

Bilingual Stipend for Spanish or Haitian Creole fluency (upon passing proficiency assessment)
