Cowlitz Family Health Center is seeking a full-time Nurse Practitioner to serve as a Multi-Site Advanced Practice Provider, supporting access and continuity of care across our clinics. This role is designed to enhance access to care, maintain continuity for patients during provider absences, and support team-based clinical operations. The APP practices within their licensed scope, delivers evidence-based care to patients of all ages, and contributes to CFHC's mission of providing high-quality, patient-centered services to the community.

  • Wage Range: $131,00.00 - $139,050.00 Annually
  • Schedule: Monday Through Friday, Shifts variable
  • FTE: 1.0

Requirements

  • Nurse Practitioner licensed to practice in the State of Washington, without restriction, probation, or limiting condition or threat thereof
  • Board certified or Board eligible in Family Practice, Internal Medicine, or Med/Peds
  • Possess a valid, unrestricted license or registration to prescribe drugs, medications, pharmaceuticals, and controlled substances
  • Never been disciplined by any professional or peer review organization, governmental agency, licensing body or hospital medical staff for any action or omission based on or related to quality of care.
  • Knowledge of evidence-based primary clinical care standards.
  • Demonstrated ability to collaborate effectively in a team setting.
  • Ability to maintain effective and professional relationships with patients and other members of the care team.
  • Strong communication skills.
  • Ability to effectively engage patients in improving and maintaining their health
  • Experience working with underserved, transient populations.
  • Proficient computer skills and experience with electronic health records

DESIRED EDUCATION, SKILLS AND EXPERIENCE:

  • Two or more years related experience providing primary care in an outpatient setting.
  • Bilingual English/Spanish.
