Job description

Psychiatric Nurse Practitioner - Long-Term Care (Adults 18+)
A Great Opportunity / Full Time
At ViaQuest we provide specialized, individualized psychiatric care to adults in long-term care settings. We serve residents ages 18 and above, many of whom live with complex mental health needs, chronic illness, or developmental disabilities. Our psychiatric nurse practitioners partner closely with each facility's long-term care healthcare team to deliver individualized, high-quality mental health care for residents. Responsibilities may include:
  • Performing face-to-face assessments and ongoing psychiatric visits with adult clients in long-term care facilities.
  • Maintaining current patient records to clearly document problems, treatment plans, actions, goals, and changes as they occur.
  • Educating and supporting caregivers and facility staff.
  • Managing and adjusting psychiatric medications to promote stability and well-being.

Requirements for this position include:
  • Graduate of an accredited Nurse Practitioner program.
  • PMHNP license required.
  • Experience in psychiatric, mental, and behavioral health services.
  • Long-term care and/or geriatric experience strongly preferred.
  • Outpatient and tele-psychiatry experience a plus.
  • Willingness to travel within assigned service area.
