Job Summary and Responsibilities

Dignity Health Medical Group - Arizona  is seeking a full-time Geriatric Nurse Practitioner to join our busy department in Prescott, Arizona

  • EMR: Cerner
  • This position will be Alzheimer’s and Dementia Care focus.
  • Project volume: 250 patients
  • Office Patients: Patients are seen in the office with some off-site home visits in the surrounding area
  • Typical work week (hours worked; hospital vs. clinic) - option to work 4 or 5 days per week. Working in clinic setting, as well as, off site visits throughout the community to accommodate the patient’s needs. Typical patient visit load of 2 new consults a day and f/u visit

Job Requirements
  • Current Board Certification
  • Valid medical license
  • Valid DEA certificate
  • 2-5 years experience in geriatrics with focus on cognitive diseases preferred
