Delivering Solutions that Matter:
  • Conduct comprehensive psychosocial assessments and develop individualized care plans to address residents' emotional, social, and discharge planning needs.
  • Provide counseling, intervention, and ongoing support to residents and their families while advocating for resident rights and quality of life.
  • Collaborate with an interdisciplinary care team to coordinate services, facilitate care conferences, and ensure resident-centered care.
  • Connect residents and families with community resources and post-discharge support services to promote successful care transitions.

Qualifications for Success:
  • Bachelor or Master of Social Work (BSW or MSW)
  • Current, unencumbered MA Social Worker license (LSW)
  • Prior experience in a long-term care setting strongly preferred
