Position Overview


Journey Nursing Services provides Private Duty Nursing, Home Caregiving, and RN Delegationthroughout Snohomish, King, Whatcom, and surrounding counties.

This position providescase management for Private Duty Nursing clients, including completing comprehensive nursing assessments in patients' homes, developing and updating individualized Plans of Care, coordinating care with physicians and interdisciplinary team members, and providing ongoing clinical oversight.


In addition, this role providesRN Delegation services, skilled nursing oversight, and direct patient careacross a variety of community-based settings, including:


  • Private Homes
  • Adult Family Homes
  • Assisted Living Communities


This role offers a unique combination ofclinical leadership, case management, nurse delegation, and hands-on nursing care, allowing nurses to build meaningful, long-term relationships with patients while practicing at the top of their license in a diverse and rewarding community-based setting.


Schedule & Work Environment

This position offers a dynamic and flexible schedule that combines direct patient care, clinical case management, and administrative responsibilities.


A typical work week consists of a combination of:

  • Scheduled Private Duty Nursing shifts
  • RN Delegation visits, including initial assessments, renewals, and supervisory visits
  • Case management responsibilities for Private Duty Nursing clients
  • Administrative time for documentation, care planning, provider communication, and interdisciplinary collaboration


Full-time positions typically include a combination of case management responsibilities and regularly scheduled Private Duty Nursing shifts, allowing nurses to maintain their bedside skills while building lasting relationships with the patients they serve.


Essential Responsibilities


  • Manage an assigned caseload of Private Duty Nursing clients.
  • Complete admission, renewal, and ongoing comprehensive nursing assessments.
  • Develop, implement, and update individualized Plans of Care.
  • Coordinate care with physicians, case managers, therapists, pharmacies, and other interdisciplinary team members.
  • Complete initial and renewal RN Delegation assessments.
  • Perform required RN Delegation supervisory visits and caregiver oversight.
  • Provide education, competency validation, and ongoing support for caregivers.
  • Deliver skilled nursing services and direct patient care as assigned.
  • Provide skilled care for medically complex patients, including when applicable:
    • Tracheostomy care
    • Ventilator management
    • Enteral feeding
    • Total Parenteral Nutrition (TPN)
    • Medication administration
    • Other skilled nursing interventions
  • Monitor patient progress and revise Plans of Care as clinical needs change.
  • Maintain accurate and timely electronic medical record documentation.
  • Participate in quality improvement, compliance, and regulatory initiatives.
  • Foster a culture of teamwork, professionalism, accountability, and clinical excellence.
  • Perform other duties as assigned.


Qualifications Required

  • Current Washington State Registered Nurse (RN) license in good standing
  • Current BLS Certification
  • Minimum one (1) year of nursing experience
  • Excellent assessment, clinical judgment, and critical thinking skills
  • Strong communication, organizational, and teaching abilities
  • Valid driver's license, reliable transportation, and willingness to travel throughout the assigned service area


Preferred

  • Nurse Delegation experience
  • Home Care, Home Health, Hospice, or Private Duty Nursing experience
  • Case Management experience
  • Experience caring for medically complex patients
