Job Description

We are looking to hire an experienced Registered Nurse (RN) to provide Professional Medication Monitoring (PM2) services to individuals with Intellectual and Developmental Disabilities (IDD) through Utah's HCBS Waiver program.

Job Duties:

  • Oversee and support all medical and healthcare-related needs in home and community-based settings
  • Record individuals' medical history, symptoms, and vital signs
  • Update charts and electronic health records (EHR)
  • Perform medical tasks including blood tests, injections, and other authorized clinical procedures
  • Implement doctor-recommended treatment plans per each individual's PCSP
  • Attend and schedule medical appointments as needed
  • Train and orient community support staff on medical protocols and individual health needs
  • Collaborate with physicians, support coordinators, DSPD staff, and agency administrators
  • Review medication packs and ensure proper administration and documentation
  • Contribute to care plan development with home managers, guardians, and medical professionals
  • Complete clinical records and service documentation daily
  • Transport individuals to community destinations as necessary

Requirements:

  • Active Utah RN license in good standing (DOPL)
  • CPR/First Aid Certification
  • US Work Authorization
  • Clearance of BCI Background Check and DSPD Registry Verification

Job Type: Full-Time and Part-Time

Pay: Competitive hourly rate commensurate with experience

Schedule:

  • Flexible scheduling available — day and on-call shifts

Experience:
