Job description

Job Title:Licensed Practical Nurse (LPN), ElderONE(Full Time, Days/Evenings)

Department:Homecare

Location:Sisters of Mercy, 1437 Blossom Road, Rochester, NY 14610

Hours Per Week:40

Schedule:Day (7:00am to 3:30pm) and Evening (3:00pm to 11:30pm) shifts. Every other weekend required.

This position has a designated home base facility and may require occasional travel to additional locations. Typically, travel is limited to one to two locations per day, with up to four sites in Rochester, NY and one in Newark, NY. Mileage reimbursement is provided.

SUMMARY

The Licensed Practical Nurse is responsible for providing direct patient care, usually under the direction of a Registered Nurse, in accordance with established plans.

RESPONSIBILITIES

Conducts and documents a thorough observation of each patient's medical status upon admission and throughout the patient's course of treatment.

Assists in the implementation of the patient's treatment plan under the direction of the RN.

Provides routine nursing services for patients as directed by the RN.

Complies with the evaluation, treatment, and documentation guidelines of the department.

Completes required documentation in an accurate and timely manner.

Provides regular patient status updates to the appropriate personnel.

Attends and participates in department/facility meetings as required.

Acts in compliance with hospital, regulatory, and professional standards and policies.

Maintains clinical competency by pursuing continuing education opportunities.

Performs other duties as assigned.

REQUIRED LICENSURE / CERTIFICATION SKILLS

  • Graduate from an accredited School of Practical Nursing.

  • Current LPN licensure in State of New York.

  • Basic Life Support (BLS) Certification from the American Heart Association (AHA).

PREFERRED QUALIFICATIONS

  • Strong communication, customer service, and interpersonal skills highly desired.

  • 1 year of experience working with a frail or elderly population preferred.
