Job description


Job Summary
We are seeking a Registered Nurse to join our team. In this role, you will primarily be creating and managing a plan of care for individual patients. You are a Registered Nurse (RN) who operates with great attention to detail and a big heart. You take pride in creating personalized care plans for patients and are able to observe and communicate effectively in your patient's best interest. Must have at least one year experience in Nursing Supervision and/or nursing administration in a licensed health care facility.

Responsibilities 
  • Coordinate with other colleagues to assess, plan, implement, or evaluate patient care plans 
  • Conducting quarterly care planning conferences for each participant enrolled
  • Provide nursing care based upon physician's orders and daily assessment of each participant's health status
  • Participating in staff education activities and providing consultation facility personnel
  • Ensuring that nursing services are provided to the participant as specified in the nursing care plan
  • Administer medications and monitor patients and manage coordination of care 
  • Document any changes in patient conditions and report on changes 
  • Manage participant medical records including periodic review for content and completeness
  • Train nursing staff and supervise all staff in the use of standard protocols for communicable disease and infection control
  • Maintain accurate records of patient care and discuss observations with the supervisor

Qualifications
  • Currently licensed Registered Nurse (RN) in state of employment 
  • Understanding the needs of the elderly
  • Demonstrate good communication skills
  • Excellent interpersonal and client care skills
  • Valid driver’s license 
  • First aid/CPR certification required 
  • Supervisory and/or management experience
