How you’ll make an impact in this role
  • Deliver compassionate, high-quality direct patient care, ensuring all clinical actions strictly adhere to organizational best practices and protocols.
  • Manage the entire patient care journey, meticulously documenting progress, executing physician and nursing orders promptly, and communicating critical updates to the multidisciplinary team to ensure continuity of care.
  • Coordinate care across all clinical disciplines, acting as a primary liaison to guarantee seamless and well-integrated patient services that lead to optimal health outcomes.
  • Facilitate comprehensive discharge planning by proactively assessing patient needs and collaborating with the broader healthcare team to ensure a safe, smooth, and successful transition home.
What minimum requirements you’ll need

Licensure / Certification / Registration:

  • BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
  • Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required. Licensure from the Wisconsin Board of Nursing OR current home state license if considered multi-state/Compact State.

Education:

  • Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
