Work Environment

This role is predominantly remote, allowing for flexibility in managing work-life balance while providing vital support to our clients. As a member of our hospice center's mental health team, you will engage with a collaborative, multidisciplinary team in a setting that prioritizes compassionate care and comprehensive support for patients and their families.

Key Responsibilities
  • Conduct comprehensive mental health assessments, including psychosocial evaluations, diagnostic interviews, and thorough risk assessments to ensure accurate understanding of client needs.
  • Collaborate closely with patients to design personalized treatment plans that respect cultural considerations and align with their unique goals and needs.
  • Implement evidence-based therapeutic interventions, including cognitive-behavioral therapy (CBT), dialectical behavior therapy (DBT), and solution-focused brief therapy (SFBT) across various formatsโ€”individual, couple, family, and group sessions.
  • Provide immediate crisis intervention services, working alongside other healthcare professionals to ensure continuity of care for clients in acute distress.
  • Maintain meticulous clinical documentation, including progress notes, treatment plans, and discharge summaries, ensuring compliance with clinic policies and regulatory standards.
  • Deliver psychoeducation to clients and their families, equipping them with strategies and tools to effectively manage their mental health challenges.
  • Collaborate with a diverse, multidisciplinary team comprising psychiatrists, psychologists, nurses, and case managers to facilitate comprehensive, interdisciplinary care for our patient population.
  • Regularly conduct outcome assessments to evaluate the effectiveness of treatment plans and make necessary adjustments to optimize client progress.
