This role is predominantly remote, allowing for flexibility in managing work-life balance while providing vital support to our clients. As a member of our hospice center's mental health team, you will engage with a collaborative, multidisciplinary team in a setting that prioritizes compassionate care and comprehensive support for patients and their families.

• Conduct comprehensive mental health assessments, including psychosocial evaluations, diagnostic interviews, and thorough risk assessments to ensure accurate understanding of client needs.
• Collaborate closely with patients to design personalized treatment plans that respect cultural considerations and align with their unique goals and needs.
• Implement evidence-based therapeutic interventions, including cognitive-behavioral therapy (CBT), dialectical behavior therapy (DBT), and solution-focused brief therapy (SFBT) across various formats—individual, couple, family, and group sessions.
• Provide immediate crisis intervention services, working alongside other healthcare professionals to ensure continuity of care for clients in acute distress.
• Maintain meticulous clinical documentation, including progress notes, treatment plans, and discharge summaries, ensuring compliance with clinic policies and regulatory standards.
• Deliver psychoeducation to clients and their families, equipping them with strategies and tools to effectively manage their mental health challenges.
• Collaborate with a diverse, multidisciplinary team comprising psychiatrists, psychologists, nurses, and case managers to facilitate comprehensive, interdisciplinary care for our patient population.
• Regularly conduct outcome assessments to evaluate the effectiveness of treatment plans and make necessary adjustments to optimize client progress.
• Stay informed about advancements in mental health care practices, integrating new knowledge into your clinical approach to enhance patient outcomes.
• Uphold ethical standards and maintain strict confidentiality regarding all patient-related information.

In this role, you will primarily work with individuals experiencing a range of mental health issues, including but not limited to anxiety, depression, trauma-related disorders, and interpersonal challenges. You will approach each case with a trauma-informed perspective, ensuring that interventions are sensitive to the unique experiences of each client. Our clients represent a diverse array of backgrounds, and your ability to connect, understand, and communicate effectively will be crucial to their therapeutic journeys.

• A Master’s degree in Social Work is required.
• Active state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.
• Proven ability to work both independently and collaboratively in a multidisciplinary team environment.
• Exceptional interpersonal and communication skills, with demonstrated ability to establish rapport with clients from diverse cultural and socioeconomic backgrounds.

• Experience in hospice or palliative care settings is preferred.
• Familiarity with advanced therapeutic methodologies or specialized training in trauma-informed care is an asset.
• Demonstrated capability in crisis management and intervention.

• Annual salary range of USD 70,000 to 100,000, dependent on geographic location, qualifications, and experience.
