Position Summary:

Vitae Healthย is seeking a dedicated Nurse Practitioner to provide high-quality, patient-centered medical care to residents in a skilled nursing facility. The NP will assess, diagnose, and manage acute and chronic conditions while collaborating with an interdisciplinary care team to improve patient outcomes and quality of life.ย 

Key Responsibilitiesย 

  • Perform comprehensive health assessments, diagnose and manage acute and chronic conditions, and develop individualized, evidence-based care plans for residents in a skilled nursing settingย 
  • Provide ongoing medical management including ordering and interpreting diagnostics, prescribing treatments, conducting medication reviews, and proactively monitoring conditions to prevent complications and reduce hospitalizationsย 
  • Deliverย timely clinical interventions in response to changes in patient status, ensuring high-quality, patient-centered care that improves outcomes and supports functional well-beingย 
  • Supportย value-based care initiativesย by focusing on quality outcomes, reducing avoidable hospitalย readmissions, optimizing cost of care, and utilizing data to guide clinical decision-making and performance improvementย 
  • Collaborate closely with physicians and interdisciplinary team members to ensure coordinated, efficient care, and participate in care plan meetings and quality improvement effortsย 
  • Facilitate smooth transitions of care across settings, ensuring continuity, reducing gaps in treatment, and supporting successful admissions and dischargesย 
  • Educate residents and families on diagnoses, treatment plans, medications, and preventative care strategies to promote engagement and long-term healthย 
  • Maintain accurate, timely, and compliant documentation while adhering to all clinical, regulatory, and organizational standardsย 

Requirements

  • Master's degree in Nursingย from an accredited Nurse Practitioner programย 
  • Active and unrestricted Advanced Practice Nurse (APN) license inย respective stateย 
  • Current DEA registration and prescriptive authority (or eligibility)ย 
  • Board certification as a Nurse Practitioner (e.g., FNP, AGNP)ย 
  • 2ย years of clinical experience preferred, ideally in geriatrics, long-term care, or post-acute settingsย 
  • Strong clinical assessment, diagnostic, and treatment planning skillsย 
  • Knowledge of chronic disease management and geriatric care principlesย 
  • Ability to deliver high-quality, value-based care focused on outcomes, cost-efficiency, and reduced hospitalizationsย 
  • Proficiency in evidence-based practice and clinical decision-makingย 
  • Ability to work both independently and collaboratively within an interdisciplinary teamย 
  • Excellent communication and interpersonal skills with residents, families, and staffย 
  • Strong organizational, time management, and documentation skillsย
