We are seeking a compassionate and skilled Home Health Registered Nurse to join our collaborative clinical team. In this role, you will provide individualized nursing care, manage a dedicated caseload, and help patients recover, manage chronic conditions, and maintain their independence at home.

What You'll Do

As a Home Health Registered Nurse, you will take primary responsibility for coordinating and managing an assigned patient caseload.

Your responsibilities will include:

  • Completing comprehensive initial and ongoing nursing assessments in patients' homes

  • Evaluating each patient's physical, psychosocial, environmental, and safety needs

  • Developing, implementing, coordinating, and updating individualized Plans of Care

  • Providing skilled nursing services in accordance with physician orders, agency standards, and applicable regulations

  • Monitoring patients' conditions, progress, medications, symptoms, and response to treatment

  • Performing follow-up visits based on each patient's clinical needs and Plan of Care

  • Educating patients and caregivers on diagnoses, medications, treatments, disease management, self-care techniques, and signs that require clinical attention

  • Coordinating care with physicians, therapists, social workers, home health aides, facilities, and other members of the interdisciplinary team

  • Communicating significant findings, changes in condition, safety concerns, or environmental risks promptly to the appropriate clinical personnel

  • Revising the Plan of Care when patient needs change and obtaining updated physician orders as required

  • Helping patients and caregivers access appropriate clinical, family, and community resources

  • Taking proactive steps to reduce avoidable hospitalizations and emergency department visits

  • Maintaining timely, accurate, and complete clinical documentation

  • Protecting patient and employee privacy and confidentiality in accordance with HIPAA requirements

  • Participating in quality-improvement, safety, and patient-outcome initiatives

  • Performing additional duties and special projects as assigned

What You'll Need
  • Graduate of an accredited nursing program

  • Current, unrestricted Registered Nurse license in the applicable state

  • At least one year of nursing experience in a clinical care setting

  • Associate Degree in Nursing required; Bachelor of Science in Nursing preferred

  • Previous home health or hospice experience preferred

  • Homecare Homebase experience preferred

  • Strong clinical assessment, communication, organization, and care-coordination skills

  • Ability to manage an assigned caseload and work independently in the field

  • Ability to collaborate effectively with patients, caregivers, physicians, and an interdisciplinary clinical team

  • Dependable vehicle, valid driver's license, and current automobile insurance in accordance with state requirements

  • Willingness and ability to travel within the assigned branch coverage area

  • Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
