Work Environment

Key Responsibilities
  • Conduct thorough bio-psychosocial evaluations, diagnosing and addressing psychosocial problems that hinder patients from reaching their optimal mental health.
  • Develop individualized treatment plans in collaboration with patients and their families, employing evidence-based therapeutic modalities, both in individual and group settings.
  • Document all social work activity in patients' medical records in accordance with departmental standards, ensuring compliance with professional ethics as guided by the NASW Code of Ethics.
  • Provide education and resources to patients and families, helping them understand and accept clinical recommendations aimed at restoring optimal functioning.
  • Consult with medical and support staff to offer insights into a patient's psychosocial needs, promoting enhanced coordination of care and improving patient experiences.
  • Foster patient advocacy efforts, focusing on self-determination to improve social support and community resources for clients.
  • Maintain a commitment to continuing education, attending relevant training, and utilizing available community resources to meet specific patient needs.
  • Collaborate with the Social Work Manager and other team members to ensure a safe and compliant environment aligned with healthcare regulations.
Patient Population / Client Focus

Our LCSW will engage with a varied client population facing a range of mental health challenges, including anxiety, depression, trauma, and relationship issues. Our commitment to an inclusive and culturally competent approach means that you will be equipped to meet the unique needs of each patient while fostering a sense of safety and respect.

Qualifications
  • A Masterโ€™s Degree in Social Work from an accredited institution and a valid LCSW license issued by the State Board is required.
  • At least one year of clinical experience in a health or mental health setting, preferably working with similar populations.
  • Strong understanding of human development stages and cultural considerations relevant to the care process.
  • Exceptional interpersonal skills, needed to establish trust and rapport with patients and their families.
  • Proficient analytical and therapy skills to effectively evaluate patients and assess treatment progress.
Preferred Experience
  • Prior experience in community health or outpatient settings is highly desirable.
  • Familiarity with telehealth platforms and remote therapy practices will enhance the ability to connect with patients effectively.
  • Experience in group therapy facilitation is a plus.
Compensation and Benefits
