Overview
Copley Hospital, located in Morrisville/Stowe VT is in search of a RN Patient Navigator to join our team at Mansfield Orthopaedics!
Mansfield Orthopaedics offers a comprehensive range of evidence-based medicine and proven treatments and techniques that consider the patients' wishes and preferences. Many of our Orthopaedic Staff and Providers are active athletes and outdoor enthusiasts, making us particularly well-suited to serve the active, 4-season lifestyle of our patients. We are sensitive to patients' desire to regain their mobility and get back to doing the things they love as soon as possible. We currently seeking a RN to join our team.
As a licensed member of the care team, at Mansfield Orthopaedics, the RN will work in an Orthopaedic specialty clinic to support the patients' plan of care. The RN will assists with diagnostic and treatment services for ambulatory patients suffering from a broad range of Orthopaedic dysfunction and is familiar with standard Orthopaedic concepts, practices and procedures. The RN will triage phone calls from patients and outside care providers and communicate with the Orthopaedic providers to facilitate patient care.
The RN will work on patient navigation in our Orthopaedic specialty clinic to facilitate all necessary care from the decision to have a joint replacement through the final post-operative visit. The RN will serve as a single point of contact for physicians, patients, and caregivers to provide resources and assistance with accessing clinical and supportive care services offer at Mansfield Orthopaedics.
We envision this postion primarily supporting our Waterbury Clinic.
This is a Full-Time position (PT available) with typical clinic hours Monday-Friday (no evenings, weekends, nor holidays).
Copley Hospital has a variety of shifts available across Full-Time, Part-Time, and Per Diem needs. Please consider applying to discuss how your availability may align.
Compensation: $75,000-95,000/year.
The posted salary reflects base compensation for candidates with limited experience. An offer may vary based on factors such as experience, education, skills, internal equity, and market data. An offer of employment may be outside of the posted range based on those factors.
Responsibilities
  • Manages self and resources effectively. Organizes multitask workload, accepts changes in assignment, utilizes supplies appropriately, requests assistance appropriately.
  • Assist with diagnostic and treatment services for ambulatory patients suffering from a broad range of Orthopaedic dysfunction and is familiar with standard Orthopaedic concepts, practices and procedures.
  • Applies critical thinking knowledge, integration skills and considers alternatives in a multitask workload. Collects integrates data from multiple sources, seeks assistance information appropriately.
  • Applies leadership skills appropriately. Coordinates care within the multi-disciplinary team, interacts in a professional manner, communicates directly, and verbalizes scope of practice for self and others.
  • Manage patients 120 day surgical journey with the assistance of our digital platform.
  • Supports the organizational values: Compassion and respect for Human Dignity, Commitment to Professional Competence, and Commitment to a spirit of service, Honesty, Confidentiality, and Good Stewardship careful administration.
  • Communicates with patients, team members, and management in an accurate, timely manner. Reports pertinent data, completes forms and charting, protects confidentiality, attends staff meetings or retrieves information from same, reads communications.
  • Identifies & applies age specific safety needs when performing job duties in the following circumstances: disaster/evacuation, transfer of patients, minimizing security risks, restraints/immobilization devices, use of equipment.
  • RN will triage phone calls from patients and outside care providers and communicate with the Orthopaedic providers to facilitate patient care.
  • Meets Copley Hospital standards related to telephone etiquette, customer service, comportment, inter intra departmental relations, confidentiality, corporate compliance, patient provider rights, UOR reporting mechanism, QI, Code of Conduct.
  • Develops and maintains human care relationships & integrates the influence of the human experience of health, illness &/or healing. Adapts care for diverse populations, assists colleagues with care delivery, and refers all concerns/issues to appropriate resource.
  • Demonstrates the unit specific skills requisite for assigned service area.
  • Integrates various aspects of specific knowledge for the patient populations served within unit area of hire: applies knowledge of expected norms in planning interventions. Demonstrates integration of knowledge, skills, and abilities for the patient populations served in unit of care.
  • Incorporates teaching in delivery of care and demonstrates competence in health promotion & disease prevention. Provides information to patient family appropriately, assesses for barriers to learning, uses colleagues as a learning resource.
  • Education, attendance, and punctuality, meetings: Participates in continuing education programs & mandatory education to maintain competence and gain additional skills & documents attendance. Completes annual review and or training i.e. HealthStream topics, mandatory education, BLS renewal, etc. Attendance & punctuality expectations met. Attends unit staff mtgs or retrieves info from the same.
  • Other duties as assigned.

Qualifications
Education: Required - Associates Degree or equivalent certification. Desired: Bachelors Degree
Experience: Required - one year's practice as Registered Nurse. Desired: 1 - 2 years experience in expanded nursing role
Licensure and certification: Registered Nurse licensed in the State of Vermont. BLS.
