Key Responsibilities

• Conduct thorough assessments and ongoing evaluations to monitor the progress and needs of clients.
• Facilitate effective communication among clients, family members, and the healthcare team to ensure comprehensive support.
• Create and share detailed plans with the clinical team that outline appointments, home visits, and observations to track client development.
• Perform home visits as necessary, to conduct in-home assessments and gather information crucial for treatment planning.
• Execute program strategies with precision to optimize client outcomes in accordance with best practices in mental health care.
• Participate in interdisciplinary team meetings and contribute insights to enhance collective effectiveness.
• Document progress notes and therapeutic interventions regularly, ensuring compliance with state and federal regulations.
• Lead skill-building sessions for team members to ensure adherence to program protocols and documentation standards.
• Conduct follow-up assessments to verify that clients' evolving needs are met and that they are progressing toward their goals.
• Maintain and update client records to facilitate easy access to information for relevant parties, ensuring confidentiality and ethical compliance.

Patient Population / Client Focus

Your role will involve working with diverse client demographics, including children, adolescents, and adults experiencing a range of mental health challenges. This includes, but is not limited to, anxiety, depression, trauma, relationship issues, and substance misuse. Your culturally competent approach will be vital in addressing each client's unique circumstances and providing a supportive therapeutic environment.

Qualifications

• A Master’s or Doctoral degree in Counseling, Psychology, or a related mental health field.
• Current and unrestricted licensure as a Professional Counselor (LPC) in the state of Texas.
• Minimum of two years of relevant counseling experience, preferably in a clinical setting.
• Proficiency with EMR systems and comfort in utilizing technology to support remote client engagement.
• Exceptional interpersonal and communication skills, capable of establishing rapport with a diverse clientele.
• Demonstrated ability to work collaboratively within a healthcare team.

Preferred Experience

• Specialization in areas such as family therapy, couples therapy, trauma-informed care, or addiction counseling will be highly regarded.
• A strong understanding of legal and ethical standards in mental health practice.
