  • Must be Board certified in Orthopedic surgery or a Truly board eligible - REQUIRED
  • Have an active license in both Iowa and Illinois - REQUIRED
  • Strong clinical and surgical expertise with a commitment to comprehensive hand and upper extremity care - REQUIRED
  • Open to graduating residents/fellows and experienced surgeons
  • Does the provider require visa support H-1B cap exempt (Note in Additional Submission Details) REQUIRED
  • Submit Self-Query NPDB pulled within 30 days from submission at time of presentation- REQUIRED
  • Active Iowa medical license
  • Active Illinois medical license
