  • Demonstrated ability to work independently and as part of a multidisciplinary team.

Preferred Qualifications:

Responsibilities:

  • Provide direct nursing care to swing bed patients, including medication administration, wound care, and monitoring vital signs.

  • Collaborate with physicians, registered nurses, therapists, and other healthcare professionals to develop and update individualized care plans.
  • Educate patients and their families regarding care plans, treatment procedures, and health maintenance.
  • Ensure compliance with all regulatory requirements, facility policies, and safety standards while maintaining patient confidentiality.
  • Assist in coordinating patient admissions, transfers, and discharges within the swing bed program.
  • Participate in quality improvement initiatives and staff meetings to enhance patient care services.

Skills:

The LVN will utilize clinical nursing skills daily to assess patient conditions, administer medications, and perform treatments with precision and care. Effective communication skills are essential for educating patients and collaborating with healthcare team members to ensure cohesive care delivery. Organizational skills help manage multiple patients and documentation requirements efficiently, maintaining compliance with regulatory standards.
