Job description

Role: Clinician, FSP DTLA

Reports to: Director, FSP Downtown

Program: 1303-FSP

Department: Health & Wellness

Location: Wellness Center - 345 S. San Pedro Street, Los Angeles, CA 90013

Setting: Not 100% Onsite

Schedule: Monday - Friday, 8am - 4:30pm

Status: Full-time/Non-Exempt/Non-Management

Summary:

The FSP Clinician is responsible for engaging members and coordinating mental and recovery services that will ensure housing stability and community integration. The FSP Clinician is responsible for maintaining the highest quality provision of direct individual and case management services.

Essential Duties and Responsibilities:

  • Provide, coordinate, and implement mental health and recovery support services with individual members.
  • Develop effective, trusting relationships with clients experiencing tri-morbid disorders, as well as the forensic population, with a focus on facilitating independence and maintenance of improved physical and mental health
  • Collaborate with each member to develop their individual service plan; review and update it upon completion of goals
  • Provide linkage to stable housing, and other supportive services as needed to assist clients to obtain housing stability, in addition to client advocacy, case management, and benefit establishment
  • Conduct screening interviews, complete housing documentation, and coordinate clients' move-in with the housing authority, property managers
  • Perform an initial comprehensive case management assessment and develop a plan of intervention that addresses their barriers, increases their income, and assists them in maintaining permanent housing
  • Encourage and promote an environment that is strength-based to assist clients in meeting their individual goals
  • Identify appropriate permanent housing options for clients, such as, subsidized housing Section 8, Shelter Plus Care, VASH, permanent supportive housing, affordable and market-rate housing, and other housing opportunities
  • Work cooperatively and cohesively with other members of the FSP team, including participation in weekly staff meetings, group clinical supervision, and trainings
  • Develop and maintain a complete, accurate, and current member file with all funder-required documents

Qualifications:

  • MSW or MFT who is BBS eligible (supervision hours are available for this position)
  • Must be registered with the board of behavioral sciences
  • At least one year of experience working with clients with mental illness, chronic health issues, and substance use disorders.
  • Demonstrated knowledge of clinical case management
  • Knowledge of community resources
  • Able to work in a high-tolerance, harm reduction model with individuals who have multiple barriers
  • Strong interpersonal and team-building skills
  • Computer literate; able to effectively use computerized database for client file management, with basic skills in Microsoft Word, Outlook, and Excel programs.
  • Current, valid CA Driver's license, reliable car, auto insurance, and an acceptable driving record.
  • Able to obtain and maintain CPR/First Aid certification

Job Description Preferred Qualifications:

  • Minimum 30 WPM typing speed
  • Bilingual in Spanish preferred
  • Experience working with homeless individuals who are living with mental illness, substance addiction, physical health issues, anda forensic population preferred

Work Environment:

  • A combination of field and office environments
  • Regularly required to sit, stand, bend and occasionally lift or carry up to 35 lbs.
  • On occasion, walk or drive to different local sites
  • Will necessitate working in busy and loud environments
  • Will be exposed to elements like cold, heat, dust, noise and odor
  • May need to bend, stoop, twist, and sit throughout the day
