• Perform thorough evaluations of patients' physical abilities and conditions based on the physician's referrals, developing personalized treatment plans that align with established therapeutic goals.
• Administer appropriate therapies and interventions, utilizing evidence-based modalities and techniques tailored to patient needs.
• Continuously assess and monitor patient progress, making necessary adjustments to treatment plans and documenting changes meticulously to comply with health regulations and best practices.
• Engage in regular communication with family members and caregivers to educate them on the rehabilitation process, providing support and resources to enhance patient compliance and understanding.
• Document treatment sessions in accordance with departmental policies and regulatory requirements, ensuring all records are accurate, timely, and complete.
• Participate proactively in departmental quality improvement initiatives, contributing insights and feedback to enhance service delivery.
• Collaborate interdepartmentally with healthcare professionals, attending case conferences and family meetings to discuss patient welfare and care strategies.

As a Physical Therapist, you will primarily work with individuals recovering from surgeries, injuries, or chronic conditions. Our patient population may include seniors requiring rehabilitation post-stroke, younger patients overcoming sports injuries, and individuals managing chronic pain. You will tailor your approach to meet the diverse needs of our clients, ensuring that each case receives the utmost attention and care. Your role is crucial in guiding these patients toward functional independence and improved quality of life.

• A Bachelor's degree in Physical Therapy from an accredited institution.
• Current licensure to practice as a Physical Therapist in the state of California, with credentials in good standing.
• A minimum of 2 years of experience in a clinical setting, preferably in home care.
• A valid driver's license and access to a reliable vehicle for travel between patient locations.
• Exceptional interpersonal skills and the ability to foster trusting relationships with patients and their families.

• Experience in home care or outpatient rehabilitation settings.
• Familiarity with pediatric or geriatric physical therapy practices is a plus.
• Proficiency in using electronic health record (EHR) systems for documentation and patient management.

We offer a competitive salary range of $110,000 to $120,000, commensurate with experience, alongside a comprehensive benefits package which includes health insurance, retirement plans, and paid time off.
