What You'll Be Doing
  • Provide skilled RN home health visits throughout the Twin Cities service area
  • Complete comprehensive nursing assessments
  • Perform wound care, medication management, injections, and other skilled nursing services
  • Educate patients and caregivers on disease management and recovery
  • Collaborate with physicians, therapists, and interdisciplinary care teams
  • Support multiple territories based on patient needs and census
  • Complete accurate documentation using Homecare Homebase (HCHB)
  • Help ensure patients receive timely, uninterrupted care

Who We're Looking For
  • Active Minnesota Registered Nurse (RN) license
  • Experience in home health, hospital, rehabilitation, or skilled nursing preferred
  • Strong assessment and critical thinking skills
  • Comfortable working independently in a field-based environment
