Job description

Job Description
The Transitions of Care/Anticoagulation Stewardship Pharmacist ensures safe, accurate, and continuous medication management as patients move between healthcare settings - such as from hospital to home, or between inpatient and outpatient facilities. The pharmacist collaborates with physicians, nurses, care coordinators, and community pharmacists to ensure seamless medication plans. They anticipate discharge barriers such as cost or compounding needs, and coordinate proactively with other teams. As Anticoagulation Stewardship Pharmacist they serve as a resource for patients on anticoagulation therapy, and work with interdisciplinary teams to develop and adjust care plans, ensuring optimal balance between preventing thrombosis and minimizing bleeding risk while building this new service.
This position serves as primary preceptor for a CentraCare PGY-1 pharmacy resident rotation as well as an APPE student rotation.
Schedule:
  • Full-time | 80 hours every two weeks
  • Days | Monday-Friday | 8-hour shifts start between 7am-8am
  • Occasional weekends and holidays

Pay and Benefits:
  • Starting pay begins at $62.10 per hour and increases with experience
    • Pay range: $62.10-$93.15 per hour

Qualifications:
  • Bachelor of Science in Pharmacy degree and/or Pharm. D.
  • Advanced training (residency, fellowship, and/or board certification) and/or previous hospital experience preferred.
  • Current licensure by the Minnesota Board of Pharmacy.
