Job Description
To perform this job successfully, an individual must be able to perform the following satisfactorily; other duties may be assigned. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
  • Coordinate with nursing partner to ensure complete coverage of care.
  • Appropriately evaluates the patient, initially and periodically throughout care.
  • Administer medications, injections, IV infusions. Perform wound care, catheter care and ostomy care following strict protocols.
  • Coordinate care plans with physicians and other healthcare professionals while maintaining accurate medical documentation within EMR/EHR systems like Epic, Cerner, or Athenahealth.
  • Provide education on disease management, medication administration, nutrition, and self-care techniques to patients and their families.
  • Monitor patients' progress through diagnostic evaluations and physiological assessments to adjust care plans accordingly.
  • Ensure compliance with HIPAA regulations and infection control standards during all patient interactions.
Requirements
  • Licensed in Colorado as a Registered Nurse or Licensed Practical Nurse without stipulations.
  • Home health experience preferred
  • Current CPR certification
  • Current First Aid certification
  • Possession of a valid state driver's license with reliable transportation and applicable car insurance, if transporting patients
