Job description

Key Responsibilities:

  • Provide direct skilled nursing care according to physician’s orders and the plan of care
  • Perform wound care, injections, medication administration, IV support (if applicable)
  • Monitor patient vitals, document assessments, progress notes, and coordinate with case managers / supervisors
  • Educate patients and families / caregivers on care plans, disease processes, home safety, etc.
  • Travel to patient homes within assigned territory; maintain charting & documentation accurately and timely
Qualifications:

  • Current California LVN license in good standing
  • At least 1 year of nursing experience; home health experience preferred
  • Valid CA driver’s license, reliable transportation, auto insurance
  • CPR certification
  • Strong communication skills; bilingual skills a plus (e.g. Spanish, Vietnamese, etc.)
  • Ability to work independently; comfortable with home visits
