Job Summary:

Seeking an experienced Home Health RN with infusion experience to provide skilled nursing care for adult patients in home settings. The RN will independently manage home visits, perform assessments, IV therapy, wound care, and patient education while completing accurate OASIS documentation.

Key Responsibilities:
  • Perform skilled home health visits (6–8 visits per day)
  • Provide infusion therapy, IV care, PICC/Midline dressing changes, and wound care
  • Conduct cardiopulmonary assessments and monitor high-risk patients
  • Educate patients and families regarding medications, disease management, and safety
  • Complete documentation and charting using BayCare systems
Required Skills:
  • Infusion experience required
  • Skilled Home Health and OASIS experience required
  • IV therapy and wound care experience
  • Experience with CHF, COPD, MI, Pneumonia, Stroke, and post-op care
  • Venipuncture, wound vac, catheter care, injections, and line access experience
Patient Population:
  • General medical and post-surgical patients
  • Cardiopulmonary and respiratory patients
  • CHF, COPD, MI, Pneumonia, Stroke
  • Patients requiring IV therapy and wound care
Requirements:
  • Active RN License
  • Active BCLS Certification
