Registered Nurse Cardiac Rehab

Facility: Ascension St. Vincent

Department/Specialty: Cardiac Rehab

Schedule:  Days | Full time;  Monday - Friday with rotating Saturdays every 6 weeks. A typical schedule is 7:00-3:30; shift time can be flexible in this role. No Sundays and no major holidays.

How youโ€™ll make an impact in this role
  • Provide individualized education and support to cardiovascular and thoracic patients regarding recovery, activity progression, lifestyle modifications, and discharge expectations.
  • Assess patient readiness for activity advancement and safely implement, monitor, and adjust cardiac rehabilitation and mobility plans in collaboration with the care team.
  • Coordinate discharge planning needs with multidisciplinary team members to promote safe transitions of care and optimize patient outcomes.
  • Monitor, document, and communicate patient condition, response to treatment, and changes in clinical status in a timely and accurate manner.
  • Develop and maintain patient-centered care plans while promoting quality, safety, and evidence-based practice throughout the continuum of care.
What minimum requirements youโ€™ll need

Licensure / Certification / Registration:

  • BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
  • Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date required.

Education:

  • Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire required.
What additional preferences we're seeking
  • Inpatient cardiac nursing experience preferred.
