Location: Dallas Metroplex, TX
Pay: $38.00$55.00 per visit (based on experience)

We are seeking a motivated and compassionate Registered Nurse (RN) to join our home health and hospice team.
Position Summary

The RN provides skilled nursing care to adult patients in their homes; coordinates care with the interdisciplinary team and serves as a primary point of contact under the Director of Nursing. This role requires strong clinical judgment, excellent communication skills, and a passion for patient advocacy.
Responsibilities

  • Provide skilled nursing care to adult home health and hospice patients
  • Perform patient assessments and develop individualized care plans
  • Coordinate care with physicians, families, and interdisciplinary team members
  • Educate patients and caregivers on diagnoses, medications, and care plans
  • Maintain accurate and timely clinical documentation
  • Participate in on-call rotation and respond to PRN/emergency visits as needed
Qualifications

  • Current Texas RN license (required)
  • Minimum 1 year of home health or hospice experience preferred
  • Current CPR certification (required)
  • Valid Texas drivers license, reliable transportation, and auto insurance
  • Ability to communicate effectively in English
  • Hepatitis consent/declination documentation
  • Strong interpersonal and organizational skills
  • Marketing experience or community outreach interest is a plus
Physical & Work Requirements

  • Ability to stand, walk, and lift patients as needed
  • Comfortable working in varying home environments
  • Potential exposure to blood-borne pathogens
  • Flexible with local travel and scheduling
