What happens after someone hangs up the phone with 988 or 211? Who reaches out when someone returns home from inpatient behavioral health care?

FirstLink is seeking a compassionate and innovative leader to serve as our next Follow-Up Manager. This role leads a growing program that provides caring contact and continued support to individuals following a crisis, behavioral health discharge, or referral for community assistance.

Rather than managing cases, our follow-up program focuses on something equally powerful: helping people feel heard, supported, and connected as they continue their recovery journey. Whether someone has just left the hospital, had a conversation with 988, or needs help reconnecting with community resources, FirstLink's follow-up services ensure they don't have to navigate the next steps alone.

We offer competitive pay starting at $25/hour, along with an excellent benefits package which includes 100% paid Health, Dental, Vision, Life Insurance, and Short/Long-Term Disability and generous PTO accrual.

What You'll Do:

As FirstLink's Follow-Up Manager, you'll lead every aspect of our follow-up services, from supervising and developing the Follow-Up Specialist team to designing workflows and strengthening partnerships with providers across North Dakota and northwestern Minnesota.

You'll oversee post-discharge referrals from hospitals, student referrals from local schools, and interaction referrals from the 988 Lifeline and 211 helpline. Using data to improve engagement, you will monitor program outcomes and help expand access to follow-up services.

You'll also serve as a mentor and subject matter expert, helping develop the team while protecting the philosophy that defines FirstLink's follow-up services: genuine human connection, not transactional checklists.

Successful candidates are compassionate leaders who enjoy building meaningful relationships with both those we serve and the community partners we serve alongside. They are skills coaches who develop their team through encouragement, direct feedback, and trust. They are highly organized, comfortable managing multiple priorities, and passionate about improving systems without losing sight of the people those systems serve.

Ideal candidates have some prior experience in behavioral health, crisis intervention, suicide prevention, or care coordination. Experience supervising a team, managing programs, developing partnerships, and measuring outcomes will help you thrive in this role.
