Care Manager RN

Location: Milwaukie, Newberg, Portland, Hood River, Willamette Falls, Seaside, and Saint Vincent, OR

Assignment Duration: This is a 13-week contract position.

Schedule: Day shift, 5x8 hour schedule from 08:30 AM to 05:00 PM. Every other weekend is required.

Job Description: The Care Manager RN is responsible for providing inpatient care management services. The role may require floating between various units based on patient care needs and will include managing caseloads effectively. A strong background in inpatient care management is necessary for success in this position.

Responsibilities:

  • Coordinate patient care and ensure adherence to care plans.
  • Conduct assessments to determine patient needs and eligibility for services.
  • Collaborate with interdisciplinary teams to enhance patient outcomes.
  • Facilitate discharge planning and transition of care.
  • Manage a caseload of patients while adhering to regulatory standards.

Required Experience / Certifications / Licensure:

  • Minimum of 3 years of inpatient care management experience.
  • Current RN license required.
  • Certification in Case Management preferred.
  • Experience with the Epic charting system is mandatory.
  • BLS certification required.
