Job description

Regional Access Point Navigator (Part Time)POSITION SUMMARY:Interact with homeless or at risk of homeless clients using Housing Problem Solving Techniques in order divert from shelter placement. Coordinate with shelters for any potential openings. Update Regional Access Point Smartsheet and claim any open beds.Update Current Living Situation and Housing assessment in HMIS (Homeless Information System), confirm and maintain program data. Perform Home Stabilization Intake paperwork and Medicaid Authorization stabilize their living situation and/or assist them to transition into permanent supported housing opportunities.
