The Physical Therapist role will be carried out onsite at our state-of-the-art Rehabilitation Center located in Bridgeport, Connecticut. This facility is equipped with modern therapy equipment and technology, allowing for innovative treatment approaches. You will work in a collaborative team environment, engaging with other health professionals to deliver comprehensive care tailored to individual needs.

• Conduct thorough Physical Therapy evaluations to assess the needs of both adult and pediatric patients.
• Develop and implement individualized treatment plans that include specific therapeutic techniques and interventions based on patient evaluations.
• Regularly reassess and document patients' progress towards physical therapy goals, adjusting treatment plans as necessary to optimize recovery.
• Complete meticulous documentation, including SOAP notes, progress summaries, and care plans, utilizing the company’s electronic health record (EHR) system within set timeframes.
• Educate patients and their families on treatment processes and expected recovery challenges to promote understanding and compliance.
• Meet established clinical utilization benchmarks and other organizational objectives to contribute positively to service delivery.
• Work alongside peers from other disciplines to facilitate a coordinated approach to patient care, especially for patients receiving multiple services.
• Act as an ambassador for Flex Health Solutions, embodying our core values and fostering a culture of excellence and empathy within the therapy team.

Our experienced team provides care to a diverse patient demographic, including children, adolescents, and adults from various backgrounds and with varying diagnoses. As a Physical Therapist, you will focus on delivering personalized treatment strategies aimed at restoring functional mobility, enhancing strength, and promoting overall wellness in patients facing physical challenges. Your role will directly impact patient outcomes through active engagement and innovative therapeutic practices.

• A Master's degree or Doctor of Physical Therapy (DPT) from an accredited program is required.
• Current and unrestricted licensure to practice as a Physical Therapist in Connecticut.
• Willingness to conduct evaluations and treatments both in-person and via telehealth platforms when clinically appropriate.
• Strong interpersonal skills and the ability to connect with a diverse patient population.
• Flexibility and adaptability in a dynamic work environment, responding effectively to the evolving healthcare landscape.
• A collaborative mindset, with a strong commitment to teamwork and patient-centered care.

• Experience in a rehabilitation or outpatient setting is highly desirable.
• Familiarity with electronic health record systems and documentation standards is advantageous.
• Prior experience working with a diverse client base is preferred.

The salary for this role ranges from USD 100,000 to 120,000 annually, commensurate with experience and qualifications. Additionally, Flex Health Solutions offers a robust benefits package that includes:
