Job description

Help deliver exceptional care beyond the office visit.

First Choice Primary Care is seeking an organized, dependable nurse to coordinate the daily operations of our Care Management programs. This role is ideal for someone who enjoys structure, consistency, and ensuring that established processes are carried out accurately every day.

What You'll Do

  • Coordinate the daily operations of Chronic Care Management (CCM), Advanced Primary Care Management (APCM), Transitional Care Management (TCM)and other care management services.
  • Maintain a smaller care management patient panel while serving as a clinical resource and coach for the team.
  • Monitor daily productivity, patient outreach, and operational dashboards.
  • Ensure documentation meets organizational and regulatory standards.
  • Support Care Coordinators by answering workflow questions and removing operational barriers.
  • Coordinate patient assignments and daily workflow.
  • Review work for completeness, accuracy, and timeliness.
  • Escalate clinical concerns to the appropriate party according to established protocols.
  • Help maintain a positive, collaborative team environment. LPN
  • Communicate operational concerns and opportunities for improvement to leadership.
  • Support operational excellence across FCPC's longitudinal care management programs as services evolve to meet patient and company needs.

Qualifications

  • Current Georgia LPN license.
  • Experience in primary care, ambulatory care, or care management preferred
  • Experience with Medicare care management programs is a plus.
  • Experience with eClinicalWorks (eCW) is preferred.
  • Strong organizational and communication skills.
  • Proficiency with Microsoft Office applications.
