As a Licensed Clinical Social Worker at Stream Mindful Medication, you will:

• Utilize your clinical expertise to conduct thorough psychosocial evaluations and create individualized treatment regimens that resonate with patients' unique needs and circumstances.
• Implement evidence-based therapeutic modalities such as solution-focused therapy, behavioral activation, and motivational interviewing to enhance client engagement and outcomes.
• Document therapy progress meticulously within our electronic health record system to track advancements and adapt treatment plans as necessary.
• Collaborate closely with primary care providers, psychologists, and other healthcare professionals to forge an integrated approach to patient care that successively addresses all aspects of a patient's health.
• Support patients in medication management, collaborating with medical staff to ensure adherence and address any challenges with side effects or treatment effectiveness.

Your role as an LCSW will encompass working with a diverse array of clients, including those facing challenges such as anxiety, depression, substance use disorders, and trauma-related issues. Many of our clients are navigating a complex healthcare landscape, and your expertise will help provide them with both clinical guidance and empathetic support. You will be instrumental in empowering clients to set and achieve their mental health goals.

Qualified candidates must possess:

• A Master's degree in Social Work from an accredited program.
• Active and unencumbered licensure as a Licensed Clinical Social Worker (LCSW) in the state of Florida.
• A minimum of one year of experience as a licensed therapist, demonstrating proficiency in providing therapeutic interventions.

Experience within a community health setting or familiarity with integrated healthcare delivery systems is highly advantageous. Additional training or certifications in trauma-informed care, addiction counseling, or specific therapeutic modalities may enhance your application.
