Job description

The Field Registered Nurse Supervisor is responsible for the direction, coordination and direct supervision of the agency's paraprofessional staff and private duty/shift nursing services. This position provides in-field professional supervision and on the job training to home health aide, personal care aides, nurses, therapists and counselors. The Field Registered Nurse Supervisor provides assessment of newly referred clients, assess health needs and eligibility for home care. Develops an individualized Plan of Care for the client based on needs and findings of nursing assessments. Initiates written Medical Orders and written reports to the clients physician. The Field Registered Nurse Supervisor performs re-assessments in the home to reassess clients needs and discuss changes in the plan of treatment with client physician/family.  

Requirements:

- NYS Licensed and currently registered as a Registered Professional Nurse in good standing.

- 1-3 years of caring for clients in an acute care setting. 2 years experience in the provision of nursing care in an acute care facility.
