About the Role
Clarvida is seeking a Family Care Coordinator (FCC) to provide case management and oversight for youth placed in certified therapeutic foster homes. In this role, you will support youth and Family Care Providers (FCPs) through consistent engagement, service planning, and behavioral support, helping ensure stable placements and positive outcomes.
This position is relationship-driven and requires regular contact with youth and foster families, strong documentation skills, and participation in an after-hours and weekend on-call rotation. You will work collaboratively with internal teams to support youth development, safety, and long-term success.
Responsibilities
● Provide case management and oversight for youth in certified therapeutic foster homes
 ● Maintain weekly contractual contact with youth on assigned caseload
 ● Support Family Care Providers with guidance, coaching, and problem-solving
 ● Participate in service planning, assessments, and skill development activities
 ● Address behavioral challenges and support placement stability
 ● Collaborate with multidisciplinary teams to coordinate services
 ● Maintain accurate, timely, and compliant documentation
 ● Participate in after-hours and weekend on-call rotation as scheduled
Requirements
● One of the following:
 – Bachelor’s degree in Social Services or a related field with at least two (2) years of experience working with at-risk youth or young adults
 – Graduate degree with a minimum of one (1) year of relevant experience 
● Valid Driver’s License, clean driving record, and active auto insurance                       ● Ability to pass state and federal background checks
