Position Summary
We are seeking a licensed Speech-Language Pathologist (SLP) to provide high-quality speech therapy services to patients in a home health setting. You will assess communication, cognitive, and swallowing needs; develop individualized treatment plans; and deliver evidence-based interventions to help patients improve function, safety, and quality of life.
The ideal candidate is patient-centered, organized, clinically strong, and committed to delivering exceptional home-based care.
Key Responsibilities
Conduct comprehensive speech, language, cognitive, and swallowing evaluations in the home
Interpret patient histories, assessments, and physician orders to develop individualized plans of care
Provide therapeutic interventions for speech, language, voice, cognition, and dysphagia as appropriate
Educate patients and caregivers on treatment strategies, safety techniques, and carryover exercises
Modify treatment plans based on patient progress and evolving clinical needs
Collaborate closely with nurses, physicians, case managers, and interdisciplinary team members
Maintain accurate, timely, and compliant documentation in the EMR
Ensure adherence to federal, state, and agency regulatory standards
Stay current with evidence-based practices and professional standards in speech-language pathology
Qualifications
Masterโs degree in Speech-Language Pathology from an accredited program
Active Florida Speech-Language Pathologist license in good standing
ASHA Certificate of Clinical Competence (CCC-SLP) preferred
Strong communication, interpersonal, and patient engagement skills
Ability to work independently and manage a home health caseload
Compassionate, detail-oriented, and committed to high-quality patient care
Proficient with computers and electronic medical record documentation
Experience with Alora Plus EMR preferred
(Preferred) Prior home health experience
