Job description

Licensed Clinical Social Worker – Integrated Behavioral Health
LCSW Primary Care- Rhinebeck, NY $7,500 SIGN ON BONUS
Position Summary 
The LCSW provides integrated behavioral health services within the primary care setting, delivering assessments, brief therapeutic interventions, and care coordination for adolescent and adult patients. This role supports improved psychosocial functioning, facilitates access to appropriate levels of care, and collaborates closely with providers and the multidisciplinary team to ensure comprehensive, patient centered care.  
Key Responsibilities 
  • Conduct behavioral health assessments and develop individualized treatment and transition plans. 
  • Provide evidence-based interventions (CBT, DBT, MI, SBIRT, mindfulness, trauma‑informed care). 
  • Manage acute behavioral health concerns, including crisis intervention and suicide risk evaluation. 
  • Support behavioral health integration in primary care through collaboration, consultation, and care coordination. 
  • Facilitate referrals to higher levels of care and connect patients to community and hospital resources. 
  • Document all care accurately and timely while adhering to regulatory and compliance standards. 
  • Participate in quality initiatives, maintain professional knowledge, and model Northwell Health values. 
Qualifications 
  • Licensed Clinical Social Worker (LCSW) in CT or NY, depending on work location. 
  • Minimum 3–5 years of clinical experience. 
  • Experience in medical social work, crisis intervention, behavioral health, or integrated care preferred. 
  • Strong clinical judgment, diagnostic skills, communication abilities, and proficiency with evidence-based modalities. 
Skills 
  • Expertise in diagnostic assessment, treatment planning, and crisis management. 
  • Knowledge of mental health and substance use conditions, including cooccurring disorders. 
  • Strong communication, documentation, and organizational skills. 
  • Ability to collaborate effectively with medical providers, care teams, and community partners.
