Position Summary
The LCSW provides integrated behavioral health services within the primary care setting, delivering assessments, brief therapeutic interventions, and care coordination for adolescent and adult patients. This role supports improved psychosocial functioning,facilitatesaccess toappropriate levelsof care, and collaborates closely with providers and the multidisciplinary team to ensure comprehensive,patient centeredcare.
Key Responsibilities
  • Conduct behavioral health assessments and develop individualized treatmentand transition plans.
  • Provideevidence-basedinterventions (CBT, DBT, MI, SBIRT, mindfulness,traumainformedcare).
  • Manage acute behavioral health concerns, including crisis intervention and suicide risk evaluation.
  • Support behavioral health integration in primary care through collaboration, consultation, and care coordination.
  • Facilitate referrals to higher levels of care and connect patients to community and hospital resources.
  • Document all care accurately andtimelywhile adhering to regulatory and compliance standards.
