Job Summary: The Veteran Services Program Manager is responsible for designing and implementing a state-wide veteran's program that supports the NorthStar Care Community (NSCC) hospice organization in providing exceptional care to veterans. This role ensures that staff and volunteers understand veteran-specific needs, military culture, and best practices for honoring veterans at end-of-life. This position fosters strong community involvement by building partnerships with veteran organizations, community groups, healthcare providers, and local agencies to increase awareness, outreach, and support services for veterans and their families.
Essential Functions:
1. Designs and develops a comprehensive statewide training program on end-of-life care for veterans using evidence-based standards and best practices.
2. Leads the implementation and integration of the veteran training program across interdisciplinary teams throughout the state.
3. Creates and maintains educational materials, learning resources, and program tools that support effective training delivery and ongoing program success.
4. Plans, coordinates, and facilitates engaging workshops, webinars, in-services, and in-person educational sessions for hospice and palliative care teams.
5. Builds and supports an internal network of staff champions to promote veteran-centered care initiatives and sustain program engagement.
6. Monitors, evaluates, and continuously improves program effectiveness through data analysis, participant feedback, and evolving industry best practices.
7. Serves as the organization's subject matter expert on veteran care, military culture, and cultural competency related to veterans and their families.
8. Establishes and maintains collaborative relationships with community partners, veteran service organizations, healthcare providers, and local agencies to strengthen outreach, education, and support services.
9. Actively participates in interdisciplinary team meetings, organizational initiatives, community events, educational opportunities, and professional development activities to support collaboration and continuous growth.
10. Represents and promotes NorthStar Care Community's mission, values, and veteran-focused initiatives through active community involvement and engagement.
11. Ensures compliance with all organizational policies, procedures, protocols, regulatory standards, legal requirements, and the organizational Code of Conduct.
12. Adheres to organizational Code of Conduct, policies, procedures, protocols and processes and all regulatory and legal requirements.
13. Adheres to the NorthStar standards to care for every person, every time, 100% of the time.
Working Relationships:
Internal: Extensive contact with administrative, departmental and team staff.
External: Contact with community agencies, facilities and other programs as needed, to promote growth and understanding of hospice care.
Qualifications:
1. Bachelor's degree in healthcare, social work, education, or related field required.
2. One (1) year of healthcare experience is preferred.
3. Must have a meaningful connection to the veteran community through personal service, VA experience, or prior work in veteran-focused healthcare or advocacy.
4. Proven experience in training design and facilitation.
5. Certification in hospice/palliative care or veteran care programs preferred.
6. Familiarity with adult learning principles and e-learning platforms preferred.
7. Statewide travel is required.
8. Must possess strong interpersonal skills and possess the ability to engage diverse audiences.
9. Must possess excellent interpersonal skills, sound judgment, effective organizational, prioritization and follow-through skills, attention to detail, tact, dependability, emotional intelligence and the ability to promote positive, constructive relationships with communication and collaboration at all levels with the ability to maintain confidentiality.
10. Ability to communicate effectively and professionally in writing and verbally to perform duties and to promote good working relationships. Demonstrated experience in public speaking required.
11. Must be computer literate, with demonstrated proficiency in Microsoft Office Applications and have the ability to effectively use technology in support of operations.
12. Proof of current tuberculin testing required; patient contact will not be allowed until tuberculin clearance is documented.
13. The physical and sensory demands of the position include vision, effective speech and hearing for extensive telephone contact; repetitive motion; traveling; driving or riding in motor vehicle; standing, sitting, walking, bending, reaching, and stretching; lifting up to twenty-five (25) pounds unassisted.
14. Must have reliable transportation to be able to travel and maintain the rigors of a busy schedule. Frequently works variable hours/days; activities and workload may require extended days.
15. Must be eligible to work in the United States.
