Position Overview

Back on Track Counseling & Consulting is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team in a full-time capacity. Work Environment

This position is a remote opportunity, allowing for flexibility and work-life balance while serving clients primarily in a detox facility setting. Key Responsibilities
  • Conduct comprehensive psychosocial assessments of patients to identify their needs, strengths, and challenges related to their mental health and substance use issues.
  • Deliver exceptional counseling and disease management services, employing evidence-based therapeutic modalities tailored to individual patient needs.
  • Develop and implement personalized care plans in collaboration with clients and interdisciplinary teams, focusing on behavior modification, coping strategies, and healthy lifestyle changes.
  • Facilitate goal-setting activities with patients through synchronous and asynchronous means to help them manage chronic health conditions and associated mental health needs.
  • Perform regular outreach to patients, adapting communication methods to support their engagement in the recovery process and encourage positive behavior change.
  • Accurately document all patient interactions, progress notes, and care plans in our electronic health record (EHR) system to ensure continuity of care.
  • Engage in continuous professional development to remain current with best practices in social work and mental health treatment.
Patient Population / Client Focus

The patients in our detox center present a wide array of challenges, primarily related to substance use and dual diagnoses. You will work closely with individuals who may be experiencing withdrawal symptoms, have co-occurring mental health disorders, or need support navigating the complexities of recovery. Our clients come from diverse ethical, socioeconomic, and cultural backgrounds, providing a rich tapestry of experiences and perspectives that you will have the privilege of engaging with.

Qualifications
  • An active state licensure as a Licensed Clinical Social Worker (LCSW).
  • A Master's degree in Social Work from an accredited program is required.
  • A minimum of three years of post-graduate experience in a mental health or outpatient setting is essential to ensure you are well-equipped to handle the multifaceted needs of our clients.
  • Strong grasp of evidence-based therapeutic approaches and techniques that promote recovery and mental wellness.

Preferred Experience

Compensation and Benefits
