Position Overview

Back on Track Counseling & Consulting is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team on a full-time basis. This remote position focuses on delivering vital mental health care within a hospice center setting, where you will play a key role in enhancing the well-being of patients and their families during the most critical times of life.

Work Environment

This role offers the flexibility of a remote work setting while being deeply embedded within a hospice care framework. As a member of a collaborative interdisciplinary team, including physicians, nurses, and allied health professionals, you will engage in consultative practices to support patients holistically. Our work hours are Monday to Friday, from 9:00 AM to 5:00 PM, allowing a structured yet supportive environment for both personal and professional growth.

Key Responsibilities
  • Conduct in-depth psychosocial assessments, employing a holistic lens to identify patientsโ€™ needs, strengths, and challenges that influence their overall health and well-being.
  • Provide comprehensive counseling services tailored to individuals, assisting them in managing chronic conditions beside their mental health needs through evidence-based techniques.
  • Collaborate with patients and the healthcare team to develop and execute personalized care plans that incorporate behavior modification strategies and coping skills.
  • Facilitate goal-setting sessions with patients, promoting self-management techniques for chronic illnesses and psychological resilience.
  • Engage in proactive outreach to patients, aligning care interventions with their personal circumstances and preferences to foster active participation in their treatment journey.
  • Maintain thorough and precise documentation of patient interactions, progress notes, and care plans in our electronic health record (EHR) system to ensure continuity of care and compliance with regulatory standards.
Patient Population / Client Focus

You will primarily interact with a diverse patient population, including individuals facing terminal illnesses and their families, who require not only clinical support but also empathetic guidance through emotional challenges. Your role will ensure that these patients receive not only symptom management but also psychosocial support crucial for improving their quality of life during this sensitive period.

Qualifications
  • A Masterโ€™s degree in Social Work from an accredited program is required.
  • A current and active state license as a Licensed Clinical Social Worker (LCSW).
  • At least three years of clinical experience in a mental health or outpatient setting, demonstrating a strong foundation in patient care.
  • A comprehensive understanding of evidence-based therapeutic modalities and best practices in social work.
  • Exceptional communication and interpersonal skills, capable of building rapport quickly with patients and families.
Preferred Experience
  • Experience working with hospice or palliative care populations is highly desirable.
  • Familiarity with interdisciplinary team collaboration and the ability to navigate complex healthcare systems effectively.
  • Proven track record of applying cognitive-behavioral therapy (CBT), mindfulness practices, or similar techniques in clinical settings.
Compensation and Benefits
