Job Summary and Qualifications

As an ER Discharge Coordinator, you will be responsible for Point of Care (PoC) Patient Bedside scheduling as part of the companyโ€™s centralized ER and Urgent Care Follow-up programs. The truest intent of the ER discharge coordinator is a bedside-initiated patient experience, with the objectives of increased contact rate, scheduling immediacy, and improved overall patient experience. You will create and maintain a positive customer experience in a patient-centered, quality-driven environment. 

In this role you will:

  • Speaks with customers, patients and stakeholders in a pleasant and professional manner; Communicates appropriately and clearly with department leadership, co-workers and customers/patients. 
  • Will schedule patients for follow-up care bedside when appropriate. 
  • Ensures performance and quality standards are met. 
  • Provides superior customer service to internal and external customers and patients; Supports the vision and strategy of the organization, positively promotes the services offered, and elevates issues to leadership as appropriate. 
  • Suggests ideas for positive changes to department policies and procedures. 
Qualifications:
  • One year of related experience is preferred.
