Hourly Pay Range: $30-$36 per hour

Direct Meds Clinical is seeking a full-time Clinical Care Coordinator to support our growing telehealth and partner clinic network. This role blends clinical support, patient coordination, and operational execution in a fast-paced, remote environment.
You'll play a key role in guiding patients through their treatment journey while partnering with providers and internal teams to deliver efficient, high-quality care.

What you'll do
  • Communicate with patients through phone calls, email, SMS, and the patient portal.
  • Deliver a compassionate, professional, and patient-centered experience during every interaction.
  • Adapt to evolving workflows and support new clinical programs as they are introduced.
  • Collaborate with providers and internal teams to support patient care, including medication questions, side effects, dosing, and general treatment support.
  • Route and escalate clinical concerns to providers when appropriate.
  • Coordinate prescription workflows with pharmacy and fulfillment teams.
  • Document patient interactions accurately and efficiently in internal systems.
  • Support care coordination across GLP-1, peptides, HRT, wellness programs, and additional clinical programs as we continue to grow.
  • Meet performance expectations for response time, quality, resolution, and patient experience.

What we're looking for
Required:
  • Active LVN/LPN license or medical office experience
  • Minimum 2 years of clinical or medical experience within the last 4 years
  • At least 1-2 years of remote or telehealth experience
  • Excellent written and verbal communication skills.
  • Strong organizational skills with the ability to prioritize multiple tasks in a fast-paced environment.
  • Comfortable working independently while remaining highly engaged in a remote setting.
  • Proficient with computers and able to learn new software and workflows quickly.

Preferred:
  • Familiarity with GLP-1s, peptides, HRT, men's health, or other telehealth treatment programs.
  • Experience with EMR, CRM, or ticketing systems
  • Preferred availability for a Thursday-Monday schedule
  • Experience working in a fast-paced, high-volume environment.

  • Dedicated, quiet workspace suitable for patient calls and remote work
