• Conduct comprehensive mental health assessments, carefully considering any co-occurring medical conditions that may influence your patients' psychological well-being.
• Collaborate with an interdisciplinary team of healthcare providers to develop and implement integrated treatment plans aimed at addressing both psychiatric symptoms and associated medical issues.
• Offer personalized individual therapy and group therapy sessions, utilizing evidence-based therapeutic approaches to foster patient growth and resilience.
• Provide effective crisis intervention and support management during mental health emergencies, utilizing techniques to ensure patient safety and emotional support.
• Educate patients about the connection between mental health and physical well-being, enabling them to take proactive steps in managing their overall health.
• Maintain detailed and confidential documentation of patient interactions, in compliance with all legal and ethical standards in mental health practice.
• Actively engage in team meetings, case discussions, and interdisciplinary care planning sessions to ensure comprehensive service delivery.
• Stay informed about the latest research, methodologies, and best practices in integrated care and mental health therapy.

Your role will center around a diverse clientele, including individuals with co-occurring medical conditions such as autoimmune disorders, chronic pain, and substance use disorders. You will support these patients in navigating their unique challenges, providing tailored resources that improve their mental health outcomes and enhance their quality of life through effective treatment strategies.

• A Master’s or Doctoral degree in Social Work, Mental Health Counseling, Marriage and Family Therapy, or Psychology from an accredited institution.
• Current and valid licensure in New York State as an LMHC, LCSW, LCSW-R, Licensed Psychoanalyst, Psychologist, or LMFT.
• Proven experience working with patients facing integrated mental and physical health challenges.
• Exceptional communication and interpersonal skills, with an emphasis on compassion and empathy.
• Competence in maintaining professional boundaries and adhering to ethical guidelines while ensuring patient confidentiality.
• Proficiency with telehealth platforms and familiarity with electronic health record (EHR) systems.

• Previous involvement in telehealth services or remote counseling environments.
