Virtual Care Coordinator

Location: New York, NY
Duration: 13 Weeks
Shift: Night Shift (11:00 PM - 7:00 AM)
Hours: 40 Hours per Week
Weekend Requirement: Every Other Weekend
Pay Rate: $26-$30/hr

Position Overview

We are seeking an experienced Virtual Care Coordinator to support telehealth, remote patient monitoring, and virtual patient care services. The ideal candidate will have telehealth experience, direct patient care experience, and strong communication skills to coordinate patient care in a virtual healthcare environment.

Responsibilities
  • Monitor patients remotely for safety and clinical status updates

  • Support telemedicine and virtual care programs

  • Triage and assign patients for virtual monitoring services

  • Collaborate with physicians, nurses, and interdisciplinary teams

  • Monitor telehealth consults, appointments, and follow-up visits using EPIC and telehealth platforms

  • Escalate patient safety concerns and clinical changes to appropriate staff

  • Provide support for telehealth and remote monitoring technologies

  • Maintain accurate patient documentation and observation records

  • Participate in quality assurance and workflow improvement initiatives

  • Assist with telehealth staff training and documentation processes

  • Analyze telehealth utilization reports and support performance improvement efforts

  • Ensure compliance with HIPAA and organizational policies

Requirements
  • Minimum 2 years of telehealth experience

  • Direct patient care experience

  • Experience with remote patient monitoring and virtual care technologies

  • Experience using EMR/EHR systems, preferably EPIC

  • Ability to work independently and collaboratively
