ESSENTIAL DUTIES AND RESPONSIBILITIES (included but not limited to): We are looking for candidates who want to use their expertise to improve health, wellness and care access for patients with chronic conditions.

We are recruiting candidates that experience with cardiology patients, able to develop purposeful relationships with patients, and can provide clear reporting to the cardiology care team. The CCM assistant has the following responsibilities: Develops a patient coaching relationship that supports the patient care plan and quality of life for the patient Is a trusted and empathic support for patients and families dealing with chronic conditions Communicates abnormal findings to the cardiology care team Monitors and encourages patient adherence to care plans and participation Effectively communicates findings to the care team Supports new patients in the enrollment process Works as a liaison for the patient with the practice and providers Identifies and escalates changes to providers to reduce events such as hospitalizations or urgent care visits Works effectively with the EMR and CCM platform Performs basic monitoring troubleshooting with patients for the CCM platform Follows clinical protocols Participates in continued team education through shared learnings Contributes to a high‐quality service for physicians and patients Stays current and complies with state and federal regulations, statutes, and company policies that impact the employee's area of responsibility JOB REQUIREMENTS AND QUALIFICATIONS High School diploma or GED 1 year's experience in remote chronic care management Experience with cardiology patients – specifically heart failure Medical Assistant (MA) or Certified Nurse Assistant (CNA) credential Experience with EMRs – specifically Athena, NexGen, GEMMS and e‐clinical works Excellent communication and multi‐tasking skills SCHEDULE – 8‐hour shifts; actual hours based on customer/patient time zone Work Location: Remote Job Type: Full‐time BENEFITS 401(k) Dental insurance Health insurance Life insurance Paid time off Vision insurance Pay is dependent on experience with CCM, experience with cardiology patients and language skills: $20.00 - $24.00 per hour CVAUSA is an Equal Opportunity Employer Seniority level Entry level Employment type Full‐time Job function Sales, General Business, and Education Industries Wireless Services, Telecommunications, and Communications Equipment Manufacturing #J-18808-Ljbffr
