You will act as a trusted guide for families, identifying and qualifying prospective patients to help them achieve their goal of living safely and independently in their own homes. Duties & Responsibilities
  • Strategic Growth: Develop and execute dynamic outreach plans to meet annual growth goals and expand our reach within the community.

  • Relationship Cultivation: Identify and nurture high-quality referral leads from healthcare providers, senior service networks, and community organizations.

  • Consultative Advocacy: Conduct heart-centered, consultative conversations with prospective patients and caregivers to align their unique needs with our comprehensive services.

  • Community Presence: Represent the organization with energy and professionalism at community events, educational presentations, and networking opportunities.

  • Team Collaboration: Partner seamlessly with the team, including enrollment staff and clinical assessors, to ensure a smooth and supportive transition for every new patient.
