Job description

Position Summary:
The Specialty Case Manager (Bilingual Spanish/English) will be a member of our Social Services department, working in support of patients in our Bronx-based health centers. Specifically- this role emphasized support for our behavioral health department. Behavioral health provides children and adults with comprehensive therapy and outpatient psychiatric support. The Case Manager provides social assistance, general screenings to identify support services needed, provides internal/external referrals, advocacy, and social services for our patients. 

Key Duties:
  • Provides case management assistance to patients including populations as assigned.
  • Provides letters to patients including letters for public assistance, housing IRS, schools.
  • Supports completion of prior authorizations.
  • Provides community based resources to the population based on site/ department/ program need.
  • Advocates on patients behalf to obtain specific resources or services including specific or specialized services.
  • Works closely with departmental coordinators and team members to provide patients with support.

Minimum Qualifications:
  • Bachelor's Degree in Social Work, Psychology, Human Services, or related field of study.
  • This is an entry level case management position, no previous experience required beyond Bachelor's Degree
  • Bilingual Spanish/English, required.
  • Team-player and willing to go the extra mile to support patients in our care.
  • Proficiency with Microsoft Office.
  • Commitment to excellence and to providing quality service to patients. 

Work Environment Type, Location, and Schedule:

Onsite, Bronx, NY, Monday to Friday 8:00 am to 5:00 pm. 


Compensation:  
$45,560 per year based on 40 hours per week.
