Position Summary

The Part-Time Case Manager provides Psychosocial Rehabilitation (PSR) and targeted case management services to children, adolescents, and adults living with mental illness.


Working under the direct supervision of the Assistant Director of Case Management and the leadership of the Chief Operating Officer, the Case Manager coordinates services that promote recovery, independence, community integration, and overall wellness. Services are provided in the clinic, community, and other approved locations in accordance with Medicaid regulations, agency policies, and the recovery model of care.


This position is ideal for individuals who are compassionate, dependable, organized, and excited to begin a rewarding career in behavioral health.


Essential Responsibilities

• Provide psychosocial rehabilitation and case management services in accordance with agency policies and Medicaid guidelines.

• Coordinate, implement, and monitor individualized treatment plans.

• Maintain an assigned caseload while providing high-quality, person-centered services.

• Complete assessments, treatment plans, progress notes, and other required documentation in a timely manner.

• Ensure services remain compliant with Medicaid, Medicare, managed care organizations, and other payer requirements.

• Monitor assigned caseloads to ensure required services and documentation remain current.

• Collaborate with medical providers, therapists, intake staff, and other interdisciplinary team members to coordinate client care.

• Participate in supervision, staff meetings, trainings, and quality improvement activities.

• Respond appropriately to client needs and crises during scheduled work hours.

• Maintain professionalism, confidentiality, and excellent customer service.

• Perform other duties as assigned.


Minimum Qualifications

Education

Bachelor's degree from an accredited college or university in one of the following:

• Social Work

• Psychology

• Sociology

• Counseling

• Human Services

• Behavioral Health

• Or another closely related field

Experience

• No previous case management experience is required. We provide comprehensive training for qualified candidates.

• Previous experience in behavioral health, social services, healthcare, or human services is preferred but not required.

• Bilingual (English/Spanish) applicants are strongly encouraged to apply.


Requirements

• Valid Texas driver's license.

• Reliable transportation.

• Current automobile insurance.

• Ability to travel throughout the community to provide services.

• Basic computer skills, including Microsoft Office and Electronic Health Record systems.
